Welcome to Urology of Indiana’s Patient Help Center.
Beginning Monday, July 6th, 2020, we are making changes to enhance and expedite the patient check-in experience and increase patient safety while visiting Urology of Indiana offices.
- All patients are now required to check in “online” prior to their appointment. This includes Telehealth and in-office appointments. The first step is to create a profile on our Patient Portal: My Medical Locker. (If you already have a patient portal profile and login, you can proceed to Mandatory Check-in). Patient appointments will be cancelled if online check-in has not been completed.
- Once you have set up your patient portal profile, you will need to begin your mandatory pre-appointment check-in process. (If you previously set up your patient portal profile, simply proceed to Mandatory Check-In.)

Our new online check-in will enable us to better manage patient flow, reduce the number of patients in the waiting room and minimize the amount of patient wait time. This will also make “social distancing” easier and more efficient. And, our providers will have access to all your current medical information at their fingertips during your appointment.
The safety of our patients, physicians and staff is very important to us. Therefore, we are asking all patients to arrive wearing a mask. We will also take temperatures of each patient upon arrival to ensure that no one coming into our offices is ill.

Need a Prescription Refill?
Once you have a profile and login for our patient portal, you will be able to perform multiple functions related to your Urology of Indiana account. This includes requesting a prescription refill. You will need your login information to access this.
Information About Telehealth At Urology of Indiana
Urology of Indiana offers telehealth visits in addition to seeing patients in our offices. This enables patients to see our providers “remotely” from the safety and security of their own home.
